What is an GCCMP plan?
The GP chronic care management program in Australia is a government initiative designed to provide additional support and access to allied health services for individuals with chronic or complex medical conditions. Under the GCCMP program, eligible patients can receive Medicare rebates for specific allied health services, including physiotherapy, podiatry, occupational therapy, and exercise physiology.
How to qualify for GCCMP physiotherapy sessions?
To qualify for the GCCMP program, patients must be assessed by their general practitioner (GP) who determines if they have a chronic medical (however some doctors and lenient than other) condition that requires ongoing management from a multidisciplinary team.
If eligible, the GP can then create a care plan outlining the patient's healthcare needs and goals, including referrals to allied health professionals.

3. Attend your physiotherapy treatment and bring along your EPC plan to your appointment
